Enter Mfg. Name & Number of units including Model #'s and SN #'s/TID #'s in the space provided below:

NOTE- If your wanting a RMA for more than 10 receivers, please fill out this page & Note the number of receivers you wanting to be repaired on this RMA & fax the needed Model #'s & SN #'s to 678-583-1170.

** Please do Not ship the Remote with the receivers - Unless the Remote has a problem!
